Registered Nurse RN Case Manager covering Worcester, Leicester, Auburn, MA. The role involves providing patient care across the entire continuum of care, particularly for Hospice service line patients in their homes. The RN Case Manager is expected to exceed patient expectations and deliver quality care. The position requires a graduate of an accredited nursing program, current RN license, at least 1 year clinical care experience, preferably in hospice or home health, nursing skills per accepted standards, ability to manage multiple priorities and documentation, and reliable transportation. The RN will report to the Clinical Team Manager.
