IDR Healthcare is seeking a travel nurse RN Hospice for a travel nursing job in Colonial Heights, Virginia.
Job Description & RequirementsPrimary Responsibilities:
\n\nConduct comprehensive assessments of high-risk members to evaluate clinical, functional, and psychosocial needs.
\n\t\n\tDevelop and implement individualized, goal-oriented care plans in collaboration with primary care providers and the interdisciplinary care team.
\n\t\n\tContinuously monitor, evaluate, and adjust care plans to ensure appropriate, cost-effective levels of care and optimal patient outcomes.
\n\t\n\tIdentify and coordinate non-medical support services (e.g., housing, transportation, and community resources) to enhance compliance with treatment plans.
\n\t\n\tFacilitate seamless transitions of care by collaborating with physicians, social workers, discharge planners, and claims professionals.
\n\t\n\tUtilize specialty resources and clinical expertise to achieve optimal patient outcomes and minimize preventable complications.
\n\t\n\tMaintain detailed documentation of assessments, care coordination activities, and measurable outcomes in compliance with organizational and regulatory standards.
\n\t\n\tPromote health education, disease prevention, and early intervention strategies to improve overall member well-being.
\n\t\n\tServe as a patient advocate by ensuring privacy, confidentiality, and respect throughout all phases of the care process.
\n\t\nQualifications:
\n\nAssociate’s or Bachelor’s degree in Nursing or a related healthcare field (BSN preferred).
\n\t\n\tCurrent, unrestricted state Registered Nurse (RN) license.
\n\t\n\tStrong analytical and clinical judgment skills with the ability to interpret complex medical data.
\n\t\n\tProven ability to work collaboratively across multidisciplinary teams and manage multiple priorities effectively.
\n\t\nIDR Job ID #471899. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
